Supporting the Capture of Social Needs Through Natural Language Processing
Social factors impact morbidity and mortality among patients. Documenting social needs in the clinical notes is currently widely done by family physicians. The unstructured format of information on social factors in electronic health records limits the ability of providers to address these issues. A proposed solution is using natural language processing to identify social needs from the electronic health record. This could support physicians in capturing structured social needs information that is consistent and reproducible without increasing documentation burden. How Do Family Physicians Document Patients Social Needs in Electronic Health Records?
Social needs are critical determinants of patient health, but their capture in clinical records began recently. A representative survey of family physicians showed that 61% of respondents document social needs using notes, with fewer using diagnosis codes or electronic forms. This preference for unstructured documentation may make it difficult to connect patients across organizations or for policymakers and planners to identify geographic variation in needs. Outcomes of A Virtual Practice-Tailored Medicare Annual Wellness Visit Intervention
Conclusion: Virtual implementation of an intervention that combined EHR-based tools with practice redesign approaches increased AWV and preventive services utilization in Medicare patients. Given the success of this intervention during the COVID-19 pandemic (when practices had many competing demands), greater consideration should be given to delivering future interventions virtually. An Estimate of Severe Harms Due to Screening Colonoscopy: A Systematic Review
Conclusions: This study is the first to estimate direct harms from screening colonoscopy, including harms that occur up to 30 days after the procedure. The risk of harm subsequent to screening colonoscopy is higher than previously reported and should be discussed with patients when engaging in shared decision making. Integrating Harm Reduction into Medical Care: Lessons from Three Models
Conclusions: All 3 sites followed 8 common themes in delivering harm reduction–informed care, most of which are consistent with the broader movement toward patient-centered care. These practices demonstrate how medical providers may overcome some of the barriers imposed by the medical model and successfully integrate harm reduction as an orienting framework toward care delivery. The Association of Hearing Loss with Hospitalization
Conclusion: Patients with HL do not seem to have higher rates of hospital readmissions. We did find high frequency of HL in hospitalized patients along with significant communication difficulties that patients had with their clinicians. Peripheral Neuropathy in Older People Is Associated with Reduced Life Expectancy
Conclusions: In this relatively healthy cohort of older primary care patients, PN detectable by physical examination was extremely common and strongly associated with earlier mortality. One possible mechanism involves loss of balance, though our data were insufficient to determine whether poor balance led to injurious falls or to less-specific declines in health. These findings may warrant further studies to determine the causes of age-associated PN and potential impact of early detection and balance improvement and other fall prevention strategies.
