Currently there is an increasing focus on quality of care delivered in the United States, especially relating to payment structure. Recent payment reform has introduced important risk-adjustment for hospital and facility payment based upon patient complexity. However, this does not currently exist for Medicare surgeon fees, which are flat reimbursements largely unrelated to patient characteristics. Podium Presentation Title: Diffuse Host Bone Marrow Edema in Six-Month MRI as a Predictor for Osteochondral Allograft Failure
Many authors have reported good to excellent clinical outcomes after Osteochondral Allograft (OCA) transplantation. Previous long-term follow-up studies demonstrated high survival rates and time to failure usually higher than 2 years after the procedure. Despite this significant amount of time, early findings to predict future OCA failure remains elusive. Diffuse host bone marrow edema (BME) is expected postoperatively during the first 3 months. The development of angular stable locking late fixation in realignment osteotomy has resulted in the ability to be more aggressive with weight bearing and rehabilitation after high tibial osteotomy. One of the downsides of some of these plate fixators is the degree of soft-tissue irritation and discomfort that can be experienced by many patients, resulting in the need for hardware removal.
