Inability to intubate because of rotator cuff injury
My wife and i have been doing some traveling, been hauling around a 33 lb child for hours. Ended up with one hell of a sprain/strain/maybe even partial tear of my rotator cuff. Currently barely able to flex, abduct, and externally rotate my L shoulder, excruciating pain with those movements. Have to work in 2 days, really starting to doubt if i could intubate, it’s my arm that holds the mac blade. So ibuprofen, Tylenol, and suck it up and hope i don’t have to intubate or just hope... Treating family for minor illness
On another thread, someone mentioned that they won’t (or shouldn’t) treat their own kid for any illness that requires more than ibuprofen. That definitely took me by surprise. A huge benefit of being an emergency physician from my standpoint is the ability to confidently treat family members for minor illnesses. I’d be happy to treat my daughters ear infection, for example, if that got her faster treatment and saved a trip to the pediatrician (just an example - not starting a debate here on... Why don't pharmacies fill my tylenol prescription?
Hello All, I'm an Oral and Maxillofacial Surgeon practicing in NY. My post-operative pain regimen usually entails stacking ibuprofen 400 Q4H or 600 Q4H and tylenol 500 Q4H, and a few oxycodones (4-12) for breakthrough pain. HMy oxycodone prescription tells patients to take the ibuprofen and tylenol first, wait 30 minutes, then take the oxycodone, to drive home the idea of breakthrough pain. The only other medication I'll prescribe is an antibiotic, in probably half the patients. What... Read more (Source: Student Doctor Network)

Fell off my mountain bike: broken collarbone.
Title says it all. Looked like this: Tylenol/ibuprofen is doing just fine. Nobody needs "percocets" (sic) for such a silly thing. (Source: Student Doctor Network)

NSAIDs
Wondering what’s peoples go to NSAID for patients, why do you like it, assuming they have tried ibuprofen and naproxen already. Or do most people find trying a different NSAID typically doesn’t help many people. (Source: Student Doctor Network)

Weird case
Did an L5-S1 interlam on a seemingly normal 31 year old guy with a central disc at the same level. Calls almost a week later with complaints of severe positional headache not relieved by fluids, caffeine, Tylenol or Advil. Saw him yesterday, says headaches are intermittent but severe and only relieved by ice and laying down. Here are the pics. Doesn’t look like a wet tap at all, but can’t deny the complaints of spinal headache. To blood patch or not to blood patch. Explained to him in... Weird case (Source: Student Doctor Network)

Enteritis / Colitis
You guys give abx to all of your enteritis / colitis patients who have normal vitals, normalish (maybe watery diarrhea max) BM's and normal labs? I usually don't in most cases...but I've had a bunch of these exact cases over the past month and many have come back to the ED after I give them bentyl, probiotics, motrin, and maybe an anti-emetic. Kind of frustrating (Source: Student Doctor Network)

Podiatrists complicated cases
I was recently accepted to all podiatry schools I applied to. Podiatry was my first goal during my undergrad and I worked with other medical specialties. However, since I started working with a podiatrist (non-surgical *preceptorship graduate*) I started lacking interested in podiatry as 50-70% of patients leave the clinic with only Motrin prescription and nothing else. Are all non-surgical podiatrists like this? I was wondering what kind of complicated cases podiatrists run into that are... Podiatrists complicated cases (Source: Student Doctor Network)
