Assessment of implementation methods in sepsis: study protocol for a cluster-randomized hybrid type 2 trial
This study will be entirely conducted in the Emergency Department at 18 sites. A seco ndary aim is to identify clinical sepsis phenotypes and their impact on treatment outcomes.DiscussionThis cluster-randomized trial, employing implementation science methodology, is timely and important to the field. The hybrid effectiveness-implementation design is likely to have an impact on clinical practice in sepsis management by providing a rigorous evaluation of the 1- and 3-h bundles.FundingNHLBI R01HL162954.Trial registrationClinicalTrials.gov NCT05491941. Evaluation of negative binomial and zero-inflated negative binomial models for the analysis of zero-inflated count data: application to the telemedicine for children with medical complexity trial
ConclusionsWe recommend that the rate of zero inflation and overdispersion in the outcome should not be the sole and main justification for choosing zero-inflated regression models. Investigators should also consider other data characteristics when choosing a model for count data. In addition, if the performance of the NB and ZINB regression models is reasonably comparable even with ZI outcomes, we advocate the use of the NB regression model due to its clear and straightforward interpretation of the results. Palliative care for persons with late-stage Alzheimer ’s and related dementias and their caregivers: protocol for a randomized clinical trial
DiscussionThe study results will determine the efficacy of an intervention that addresses the extraordinary public health impact of late-stage ADRD and suffering due to symptom distress, burdensome treatments, and caregiver strain. While many caregivers prioritize comfort in late-stage ADRD, shared decision-making is rare. Hospitalization creates an opportunity for dementia-specific palliative care, and the study findings will inform care redesign to advance comprehensive dementia-specific palliative care plus transitional care.Trial registrationClinicalTrials.gov NCT04948866.
