"Functional illnesses." What is your approach?
By "functional" illness, I'm referring to and lumping together a bunch of diagnoses: POTS, fibromyalgia, mast cell activation, chronic fatigue, multiple drug intolerances, EDS III, chronic itch, chronic pain, etc. (maybe IBS and interstitial cystitis and others I didn't list). Some of these might exist as a true organic disease in some patients but most of the time it's code talk for underlying psych issues. We have these patients in all of our varying specialties. Hi,
I am a pain physician in hospital based practice. Recently I got referred with two very young patients with interstitial cystitis who have failed all conventional treatment which includes bladder distention, instillation and others. I would like suggestions and inputs, as I am going to do trial of spinal cord stimulator for interstitial cystitis. Patients are interested themselves to go for spinal cord stimulator trial for interstitial cystitis due to poor quality of life.

NBME Peds Q really bothering me, IgA Nephropathy vs PSGN
13 year old boy was brought in for a routine well-child exam. No history of prior serious illness. During the past 2 years he's complained of brown urine whenever he's had a cold. He has no complaints of urgency, dysuria, or incontinence. PE is unremarkable. Lab studies show:
Serum:
BUN: 15
Cr: 0.9
C3 90 mg/dL (nl 83-177)
Urine:
Blood 4+
Protein 3+
A) Hemorrhagic cystitis
B) HUS
C) IgA nephropathy
D) Nephrotic Syndrome
E) PSGN
The answer is supposed to be E, but I can't seem to...
