Help with a physiology-clinical scenario question
First off - This was NOT my case. I came on to relieve the call team and they were finishing up the case that I am about to describe - so the fine details are lacking. As I understand it..... a 19 y/o ruptured appendix some days before, IR drain not working...patient getting sicker and sicker...decision to OR for laparoscopic appy. Patient has been tacchycardic, but good urine output and no hypotension. Induction is smooth with no hemodynamic compromise (so i am told...i'm not sure if... We have a doc in our group that has “called out” for things like a gastroenteritis and pneumonia. As horrible as it is I have found it annoying at times because it puts everyone under a lot of stress to cover her. It seems to be an unspoken rule in my group that you don’t just “call out” because you’re sick. “In the hospital” type of sick is different (although one of my coworkers did work through appendicitis and then admitted himself after his shift, but I digress). Code airway. Post-op thyroid. What do you do?
I'm wondering what you would all do. You have just taken a patient off the table in emerg. theatre following I&D of an abscess. You're walking out to consent the lap appendix in holding bay. You get a "Code airway" page to a geries ward. You get there and find a post-op thyroid patient (5 hours post-op) tripodding in blind panic while a surgical intern is trying to get them to lie flat so they can take a blood pressure. Is this technically sepsis?
Non-perforated appendicitis and no abscess, + leukocytosis, mild tachycardia (< 110), lactate 2.5. Normal BP. Pt looks extremely well. Asking cause I'm reviewing potential sepsis fallouts (thanks CMS!). I don't believe this meets criteria but was diagnosed as sepsis by admitting physician. Should I take late-summer MCAT and still apply this cycle?
Hi everyone, I was signed up for a mid-May MCAT, but ended up canceling when I had to get my appendix removed (great timing, I know ). An MCAT seat just opened up over here for July 30 though! Should I take it and still plan to apply this cycle? My stats: a little over 3.9 sGPA and cGPA 516 on the AAMC practice a week before my test day, 517 was my highest full length (NS) Plenty of clinical volunteering and shadowing (spans 3 years) Extracurriculars are fairly good I'd say- 2... Do PDs have my old patient records from 5 years ago?
A bit of a paranoid question (I'll try my best to stop asking so many questions, i promise!) I've decided to do a rotation at this hospital in my hometown (Austin). Say 5 years ago I was hospitalized at this hospital for appendicitis and a complication due to a medical error happened. I never filed a lawsuit or anything because the caregivers were all very genuinely good people.
