Untangling diffuse CAD: Light at the end of the tunnel?

A growing number of patients with diffuse coronary artery disease (CAD) are not considered candidates for revascularization, as the complexity of this condition is often an inhibitory factor for both percutaneous coronary intervention (PCI) and coronary artery bypass graft surgery (CABG). As such, these patients are frequently treated by means of optimal medical therapy (OMT). There is no general consensus at present how to define diffuse CAD; different investigators propose different criteria based on angiography, pressure ratios like fractional flow reserve (FFR), and vessel diameter (1,2).
