A Massive Penile Tumor

A 58-year old man presented with a large penoscrotal mass. Physical examination demonstrated a massive, cauliflower-like, firm tumor appearing to arise from the left hemiscrotum deviating his penis to right as well as palpable inguinal lymph nodes (Figure 1A). Computed tomography showed a 16 cm x 8 cm mass arising from the left hemiscrotum involving the base of the penis, and bilateral inguinal and pelvic lymphadenopathy but otherwise without distant metastatic disease (Figure 2). Biopsy demonstrated scrotal intraepithelial neoplasia grade 2/3.
