A Masquerader of neonatal persistent pulmonary hypertension

A neonate was born at 37+2 weeks weighing 2444 grams without any birth asphyxia or difficult transition. She was roomed in with mother and started on breastfeeding. At 57 hours of life, she was noted to be lethargic and feeding poorly. On examination, she had tachypnea, tachycardia, pallor and hepatomegaly with a grade 3 ejection systolic murmur suggestive of congestive cardiac failure. She was shifted to neonatal intensive unit immediately, her saturation was 93-97% without significant pre- and post-ductal differences, well palpable femoral pulses and age-appropriate blood pressure records in all 4 limbs.
