A wrinkle in time to antibiotics in sepsis: when should ONE hour be the goal?

In 2020, the Surviving Sepsis Campaign (SSC) recommended administration of broad-spectrum antibiotics to children with septic shock within one hour of shock recognition. For children without clinical signs of shock, antibiotics are recommended as soon as possible after an expedited evaluation supports presence of sepsis-associated organ dysfunction, but not beyond three hours from initial suspicion of sepsis.1 Given the high prevalence of sepsis globally, coupled with reported mortality rates of up to 25%, evidence-based recommendations informing best practices for sepsis management are critically important.
