Treating the pylorus in gastroparesis: The new riddle wrapped in the ultimate enigma?

Gastroparesis is frustrating for physicians who have to contend with at least 3 commonly encountered causes (idiopathic, diabetic, postsurgical), multiple mechanistic components (extrinsic or intrinsic neuropathy, myopathy, inflammatory), diverse pathophysiological conditions resulting in similar symptoms (emptying, accommodation, hypersensitivity), lack of generally available diagnostic tests, and incomplete response to current treatments.1 Although this theory is still controversial, there is evidence that delayed gastric emptying is associated with symptoms, and prokinetic improvement of gastric emptying is associated with reduced symptoms.
