Another case discussion for the experts here on this forum. 75 year old with atrial fibrillation on Eliquis, brought in with fractured hip. Orthopedic surgery books the procedure 3 days later (unclear why delayed) for hemiarthroplasty. Labs are fairly OK, nothing absurd. Hct 28, On TTE 1 day ago, there is large pericardial effusion (unknown etiology) with evidence of right atrial collapse, right ventricle inversion, respiratory variations. Diffuse hypokinesis, with LVEF 35%. However...
