A Pearl in the Oyster Is Not Common, but Still Precious

I worked at a tertiary referral centre for many years, and once or twice a year we treated patients with isolated mesenteric artery dissection (IMAD) of the superior mesenteric artery (SMA). When we wrote the 2017 ESVS Mesenteric Guidelines1 we realised that all the important publications in this field were from Korea, Japan, and China. The reason why IMAD is more common in Eastern Asia is unclear. Both genetic and environmental factors may explain this great difference in incidence.
