Dr Emad Kandil (New Orleans, LA): I think we need to be very careful about the issue of active surveillance. Look at what we learned from prostate cancer. If you tell patients that it's not a big deal and just to follow up, 20% will stop seeing you after one year. That's what happened with prostate cancer. When you look at the Kuma data, they showed also that ideally you should do CAT scans in order to see extrathyroidal extension. Are we really going to order a CAT scan for every thyroid nodule?
