Prone posterior retro-peritoneoscopic access to the kidney in children – the controlled optical trocar approach

We present a video detailing the technique of optical access into the retroperitoneum. We have used this technique in 35 children of ages ranging from 6-months to 14-years, mainly nephrectomy and nephroureterectomy, without any operative complications or conversions. Potential advantages include safety, ease, control, accuracy of being in the right layer or space, reproducibility, reduced gas leak around the port as it is quite a snug fit with this technique of introduction not requiring to tighten the tissues around the port and lastly, reduced risk of surgical emphysema.
