We want to thank Jacques et  al1 for their comments on our recent review on the role of endoscopic mucosal resection (EMR) and endoscopic submucosal dissection (ESD) for colorectal neoplasia. Currently in the West, piecemeal EMR remains the first-line treatment for most large benign colorectal polyps, whereas ESD has been tra ditionally reserved for those lesions suspected of harboring superficial submucosal invasion. The main drawback of this approach, as alluded to in their commentary, is our inability to accurately predict submucosal invasion based on our current classification systems, and that the prevalence of occu lt advanced neoplasia may be significantly underestimated.
