Biomarkers for Adult-onset Still's disease: up-to-date. Expert Rev Mol Diagn. 2019 Jun 28;: Authors: Ahn MH, Kim HA Abstract Adult-onset Still's disease (AOSD) is a rare systemic inflammatory disease characterised by a high spiking fever, arthralgia, lymphadenopathy, and hepatosplenomegaly. The clinical manifestations of AOSD range widely from mild constitutional symptoms to life-threatening complications, including fulminant hepatitis and macrophage activation syndrome. The heterogeneous symptoms and variable disease course makes the diagnosis of AOSD difficult, and the prognosis extremely difficult to predict. Therefore, a reliable biomarker is needed for diagnosing and predicting the prognosis of AOSD. Here, we considered candidate biomarkers in patients with AOSD. S100A8/A9, S100A12, and IL-18 have all been regarded as promising candidate biomarkers in AOSD, but this study instead revealed limitations regarding these biomarkers, as well as their clinical usefulness in AOSD. Issues include a lack of confirmed clinical utility, and lack of an analysis of their cost effectiveness in comparison with existing laboratory tests. A better understanding of the pathogenic mechanisms underlying AOSD may lead to the identification of new biomarkers.
