A 78-year-old man with syncope
A 78-year-old man came to the emergency department after a syncopal episode while shopping with his wife. The wife indicated that he suffered from myocardial infarction 2 years ago and complained about dizziness for about 2 weeks. His 12-lead electrocardiogram (ECG) recorded in the emergency department is shown in Figure 1A. In view of the history and the 12-lead ECG, a pacemaker lead was placed into the apex of the right ventricle. Shortly thereafter Figure 1B was recorded.
