In Reply to Yadav and Gupta, and Hannoun-Levi et  al

To the Editor: We thank Drs Yadav and Gupta for their interest in our trial of 3-fraction compressed accelerated partial-breast irradiation (APBI) delivered with brachytherapy applicators1,2 and his letter entitled “Radiation Therapy De-escalation or Over-treatment?” regarding our report. We would like to be clear that although partial breast radiation therapy does represent a de-escalation in the scope of treatment (relative to whole breast radiation therapy), we do not view our compressed dose schedule a s a “de-escalation”—rather the dose being delivered is expressly calculated to be bioequivalent to the standard APBI schedules.
