Factitial Panniculitis Associated With Cosmetic Filler

A 47-year-old woman presented with a 5-year history of multiple hospitalizations for recurrent cutaneous ulcers and nodules of the abdomen, buttocks, and arms starting 1 year after liposuction and fat grafting of her abdomen and hips. She had had extensive evaluation, including biopsies of multiple affected areas for histopathological examination and tissue cultures for bacteria, fungi, and mycobacteria. No specific diagnosis was rendered, and results of tissue cultures were negative. However, because concern for nontuberculous mycobacterial infection (NTM) was high, the patient was treated empirically with imipenem, linezolid, and clarithromycin for 12 months, without benefit.
