A Pancreatic Lymphoepithelial Cyst With Floating Balls

An asymptomatic 60-year-old man was referred for the evaluation of a 70-mm pancreatic lesion. Blood tests, including carbohydrate antigen 19-9, were normal. On T2-weighted magnetic resonance images, the lesion appeared slightly hyperintense with inhomogeneous content. Endoscopic ultrasound revealed a cyst with a regular wall and central thickened septum, containing multiple round, echoic, floating balls (Figure  1A), avascular after contrast injection (Figure 1B). Only 10 mL of yellowish-brown and very thick fluid were slowly aspirated through a 19-gauge needle.
