Chronic Multifocal Paracoccidioidomycosis: Pulmonary and Cutaneous Involvement

A 50-year-old man presented with a 14-month history of dyspnea, cutaneous ulceration on the labial commissure, and an 18-kg weight loss since the onset of the condition. He had been a smoker and an alcoholic for the last 35  years. Positive findings on physical examination included an erythematous infiltrated plaque with an ulcer on left labial commissure (Figure 1), an ulcer with hemorrhagic dots on the first left toe (Figure 2), and respiratory crackles in the upper two-thirds of both lung fields.
