The CASTLE-AF (Catheter Ablation versus Standard Conventional Therapy in Patients with Left Ventricular Dysfunction and Atrial Fibrillation) trial by Marrouche et  al (N Engl J Med 2018;378:417–427, PMID 29385358) randomized patients with atrial fibrillation (AF), low left ventricular ejection fraction (LVEF), and heart failure (HF) symptoms to either ablation for AF or conventional medical therapy in the form of rate or rhythm control. All patients were o n guideline-directed therapy for HF that was adjusted and optimized during a 5-week run-up period to randomization.
