TO THE EDITOR: There have now been several case reports and a recent case series by Bingham et al.1 regarding lithium toxicity following Roux-en-Y gastric bypass surgery.2,3 These cases have routinely examined the relationship only in the perioperative period. It is important to note that in the discussions of these cases they make mention of an in vitro experiment that shows the dissolution of unaltered lithium carbonate capsules to be about double in the postgastric bypass environment as compared to pregastric bypass environment.
