Mandated coverage for fertility preservation featured in NEJM

(Care New England) This summer, it was announced that Rhode Island became the first state to pass a law explicitly requiring coverage for fertility preservation prior to gonadotoxic medical therapy, treatment that could directly or indirectly cause infertility. A perspective on this mandated coverage in Rhode Island and similar legislation in Connecticut has been published in the October 26, 2017 edition of the New England Journal of Medicine.

Discussion Cryptorchidism is the failure of one or both testes to descend from the abdomen into the scrotum. Congenital undescended testis (UDT)is common in young infants (1-4% in term infants and 45% in preterm infants) in that the testes will be palpable but remain high, but most testes will descend by 3-6 months and by 9 months of age only 1% remain undescended. Lots of infertility clinics call themselves infertility clinics because they treat infertile patients, and this makes a lot of sense. After all, cancer specialists treat cancer, and cardiologists treat patients with heart disease, so this is quite reasonable.One the other hand, a lot of infertile patients object to this . They say that these are pro-fertility clinics, because they are helping patients to have a baby . Since they promote fertility, why do they continue to use such a negative name ? Why not call them fertility clinics or pro-fertility clinics – wouldn’t this be much more positive and hopeful...

ConclusionThis study is at the best of our knowledge the first nationwide survey to assess the state of the art in oncofertility management. It highlights inadequate management of fertility preservation for female patients in France. Physicians reported lacking knowledge and tools that would allow them to provide patients with appropriate information. A better collaboration between cancer and fertility centers needs to be organized in France as already organized in other countries.

Infertility has long-term effects beyond the inability to conceive. Infertility may be a harbinger of future disease and altered longevity. Understanding the mechanisms of how fertility is a window to health opens new avenues of important investigation addressing interdependent physiologic processes. Identification of healthy persons at risk for future cardiovascular disease, diabetes, or cancer could allow a change in reproductive medical care by altering long-term surveillance and reduction of risk.
