What do we know about insight into illness and its association with the underlying biology of schizophrenia?

Poor insight into illness is highly prevalent in schizophrenia patients (Amador et al., 1994; Amador and Kronengold, 1998; David, 1990; Sartorius et al., 1972) and has been associated with multiple clinical outcome measures, most notably treatment nonadherence (David et al., 1992; McEvoy et al., 1989; Perkins, 2002, Amador et al., 1993). Efforts to improve insight have been ineffective or at best partially effective, which has aroused significant interest to investigate neural substrates for impaired insight in schizophrenia in an effort to develop better treatment strategies.
