[Comment] Glyburide for cerebral oedema: could an old dog have a new trick?

Swelling of a large brain infarct portends transtentorial herniation and death. Yet, there are few effective prevention and treatment options. Reactive medical treatments, including osmotic drugs (eg, glycerol and mannitol) and corticosteroids, do not improve survival and functional outcome.1 –3 Reactive decompressive craniectomy improves survival and functional outcome in some patients, but increases survival with major disability in others, and appropriate patient selection is difficult.4
