Collateral coronary flow in the interventricular septum detected on intracardiac echocardiographic imaging

A 72-year-old man with severe coronary artery disease and coronary artery bypass grafting complicated by ischemic cardiomyopathy and ventricular tachycardia (VT) was referred for redo VT ablation. Coronary angiography (CA) before the ablation procedure showed a patent left internal mammary artery to the left anterior descending artery graft and occluded venous grafts to the obtuse marginal branch and right coronary artery (RCA) with collateral branches supplying the RCA and circumflex artery.
