Retinal hemorrhages with Valsalva during laser photocoagulation in previously treated ROP

An 8-month-old girl born at gestational age 22+5 weeks and birth weight 470 grams received three intravitreal anti-VEGF injections for recurrent Type 1 ROP at postmenstrual age (PMA) 32 weeks, 40+3 weeks, and 50 weeks. At 60 weeks PMA, she had persistent posterior vascular tortuosity and extensive peripheral avascular retina. The decision was made to perform retinal laser photocoagulation under general endotracheal anesthesia.
