We appreciate the interest of Fujiyoshi and Teshima1 in our randomized trial2 comparing conventional myotomy (CM) and oblique fiber-sparing myotomy (OFS) for achalasia cardia. The incidence of significant reflux esophagitis ( ≥LA B) was not statistically different between the 2 groups. Fujiyoshi and Teshima1 postulate that oblique fibers were possibly spared in both groups, resulting in a similar incidence of post–peroral endoscopic myotomy (POEM) reflux esophagitis in our study.2 Additionally, they hypothesize that in real-world clinical practice, oblique fibers are dissected at the collar part of the gastroesophageal junction (GEJ), thereby predisposing to reflux.
