We read with interest the letter from Vege et  al1 about our recent population-based cohort study describing the natural history of inflammatory pancreatic diseases using a multistate disease model.2 We fully agree that one of the key observations of our study is the finding that more than one-half of patients with chronic pancreatitis have no previous history of acute pancreatitis. This is an important finding because it challenges the sentinel acute pancreatitis event (SAPE) hypothesis, a prevailing theory regarding the progression of inflammatory pancreatic diseases.
