An unruly case of functional two-to-one atrioventricular block

A newborn male was delivered via cesarean section at term due to acute fetal distress and fetal bradycardia, necessitating emergency pacemaker implantation. Physical examination revealed dysmorphic facial features and syndactyly in both upper and lower limbs. An electrocardiogram (ECG) showed a two-to-one atrioventricular (AV) block and a prolonged QT interval, along with biphasic T-waves in V3 (Figure 1A). During hospitalization, the infant experienced multiple seizures and episodes of hypoglycemia.
