Natural Evolution of Pancreatic Serous Cystadenoma on Endoscopic Ultrasound

A 48-year-old man was referred for endoscopic ultrasound (EUS) for assessment of a cystic lesion in the pancreatic body. The lesion was found incidentally on sonography performed for evaluation of renal colic. EUS showed a 26  × 22 mm cyst with a few septations and no solid component (Figure A). Fine-needle aspiration showed a carcinogenic embryonic antigen level of 0.53 ng/mL, and an amylase level of 100 U/L in the cyst fluid. Cytology was negative for malignancy. On follow-up EUS after 2 years, the size of the lesio n had not changed, however, the septations had become thicker (Figure B).
