'How Complex Systems Fail' and mental healthcare system design

I recently ran across this short summary of considerations for 'complex systems' in healthcare and was struck by how relevant some of the points were in relation to attempts to design complex systems to, say, 'prevent suicide,' especially considering the role of health care provider.
