Is emergency and salvage coronary artery bypass grafting justified? The Nordic Emergency/Salvage coronary artery bypass grafting study ADULT CARDIAC
CONCLUSIONS Early mortality in patients undergoing emergent and salvage CABG is substantial, especially in salvage patients. Long-term survival is acceptable in both emergent and salvage patients. Life-saving emergency and salvage CABG is justified in most patients but salvage patients have dismal prognosis if cardiac massage is needed during sternotomy.

Multi‐modal CT in acute stroke: wait for a serum creatinine before giving intravenous contrast? No!
ConclusionsThe incidence of CIN is low when MMCT is used routinely to assess acute stroke patients. In this population, CIN was a biochemical phenomenon that did not have clinical manifestations, cause chronic kidney disease, require dialysis, or negatively impact on 90‐day mRS outcomes. Renal profiling and waiting for a baseline serum‐creatinine are an unnecessary delay to emergency reperfusion treatment.

Conclusion In this pilot study of patients with moderate renal insufficiency undergoing cardiac catheterization, with-holding ACEI/ARB resulted in a non-significant reduction in contrast-induced AKI and a significant reduction in post-procedural rise of creatinine. This low cost intervention could be considered when referring a patient for cardiac catheterization.

Effects of Withdrawing Versus Continuing Renin-Angiotensin Blockers on Incidence of Acute Kidney Injury in Patients with Renal Insufficiency Undergoing Cardiac Catheterization: Results from the CAPTAIN Trial
Conclusion In patients with moderate renal insufficiency undergoing cardiac catheterization, withholding ACEI/ARB reduced (without statistical significance) the incidence of contrast-induced AKI. Additionally, a lower rise in post procedural creatinine levels was also noted. This low cost intervention could be considered when referring a patient for cardiac catheterization.

CONCLUSIONS Moderate-to-deep hypothermia combined with targeted reconstruction of the AKA provided satisfactory outcomes with thoraco-abdominal aortic repair, particularly for spinal cord protection.
