Impact of NP follow-up calls on reducing 30-day readmissions in patients with stroke
Abstract: Hospital readmissions are major contributors to increased healthcare costs and lower quality of life. Despite advanced stroke care, patients who have experienced a stroke require ongoing follow-up care to prevent complications and hospital readmissions. We evaluated the impact of NP follow-up calls, providing another level of expertise to promptly identify new symptoms and complications, on readmission rates in patients who have experienced stroke.

Patient Pathways During Acute in-Hospital Stroke Treatment: A Qualitative Multi-Method Study
Discussion:Our results show that most direct staff-patient interactions are focussed within one phase, with a smaller number of interactions extending to other phases, and no professional (group) with direct patient interactions cover more than two phases of the acute stroke pathway. Discussion Disparities in readmissions between Black and White stroke patients may be linked to the level of nurse staffing in the hospitals where they receive care. Tailoring nurse staffing levels to meet the needs of Black ischemic stroke patients represents a promising intervention to address systemic inequities linked to readmission disparities among minority stroke patients.
