Preischemic Neuroprotective Effect of Minocycline and Sodium Ozagrel on Transient Cerebral Ischemic Rat Model.
This study showed that single doses of minocycline and ozagrel prior to stroke had neuroprotective effects. These agents will be useful not only in post-stroke therapy but also in stroke prevention in several cerebrovascular procedures like carotid endarterectomy, bypass procedure, endovascular angioplasty, thromboembolectomy or thrombolysis. Conclusions— Among individuals undergoing carotid artery revascularization from a large sample of US hospitals, CAS was associated with higher risk of perioperative mortality, stroke, and unfavorable discharges compared with CEA for all ages and clinical presentations.

Picking the Good Apples: Statistics Versus Good Judgment in Choosing Stent Operators for a Multicenter Clinical Trial Clinical Sciences
Conclusions— The low periprocedural event rates in the trial suggest success in separating skillful operators from less skillful. However, it seems unlikely that statistical assessment of event rates in the lead-in contributed to successful selection, but rather successful selection was more likely because of peer review of subjective and other factors including patient volume and technical approaches. Long-Term Follow-Up Study of Endarterectomy Versus Angioplasty in Patients With Symptomatic Severe Carotid Stenosis Trial Clinical Sciences
Conclusions— The long-term benefit–risk balance of carotid stenting versus endarterectomy for symptomatic carotid stenosis favored endarterectomy, a difference driven by a lower risk of procedural stroke after endarterectomy. The Cost Per Stroke Prevented (CPSP) from Symptomatic Carotid Artery Stenosis in Ontario, Canada (S51.006)
CONCLUSIONS: The CPSP if done within the recommended time-frame for our virtual population would be $25,326. Recent data from Ontario demonstrates that between 8-36% of carotid artery revascularizations occur within the suggested timeframe. Most carotid artery revascularizations in Ontario have a CPSP of $75,978 (median time for revascularization is between 1-3 months). This represents a clear inefficiency in the system. Further research and government wide initiatives could be aimed at increasing the awareness of the CPSP in order to facilitate policy makers and other stakeholders to develop pathways to ensure prompt trea...

Predictors of Acute and Persisting Ischemic Brain Lesions in Patients Randomized to Carotid Stenting or Endarterectomy Brief Reports
Conclusions— Stenting caused more acute and persisting ischemic brain lesions than endarterectomy. However, the rate of conversion from acute to persisting lesions was lower in the stenting group, most likely attributable to lower acute lesion volumes.
