A Rare Cause of Life-Threatening Upper Gastrointestinal Bleeding
Question: A 66-year-old woman presented to the hospital with large-volume hematemesis and melena. Her past medical history was significant for an episode of native valve infective endocarditis secondary to a skin infection 4 years earlier, with an associated embolic stroke of the left middle cerebral artery. Her daily medications included aspirin and atorvastatin. She denied any intake of other non-steroidal anti-inflammatory drugs. She had no history of peptic ulcer disease or liver disease. On arrival to the hospital, the patient ’s blood pressure was 90/60 mm Hg, with a heart rate of 140 beats/min.

Burden of illness of osteoporotic fractures compared to other serious diseases among ibd patients
Inflammatory bowel disease (IBD) patients are off and on treated with steroids which is associated with increased risk of osteoporosis and fragility fractures. We conducted an analysis to study the burden of osteoporotic fractures compared to acute myocardial infarction (AMI) or stroke in hospitalized patients with IBD.

Intestinal Ischemic Manifestations of COVID-19
We read with interest the article by Norsa et  al1 on intestinal ischemia in patients with coronavirus disease 2019 (COVID-19). They further highlight the coagulopathy known to cause vascular obstructions in patients with severe COVID-19, in whom microthrombi are typically found in the lung circulation; however, also myocardial infarction and ischemic stroke were reported, particularly in the late phase of the disease. The patients reported by Norsa et al had either small or large bowel ischemia, splenic infarct, or pulmonary thromboembolism, which were fatal in 4 of the 7 cases.

Collateral Effect of Coronavirus Disease 2019 Pandemic on Hospitalizations and Clinical Outcomes in Gastrointestinal and Liver Diseases: A Territory-wide Observational Study in Hong Kong
As of July 22, 2020 there were more than 14.9 million cases and 616,000 deaths due to coronavirus disease 2019 (COVID-19) worldwide.1 Medical resources were shifted to management of infected patients, and declines in hospitalizations for acute coronary syndrome and stroke were observed.2,3 The world is now being exposed to a third wave of the pandemic, and the disruption to endoscopy service is considerable, leading to potential delays in the diagnosis of cancers and management of gastrointestinal (GI) bleeding.

We thank Drs Dai, Jiang, and Sun for their comments. We agree that assessing the risk of ischemic stroke among users of proton pump inhibitors (PPIs) with and without coadministration of clopidogrel would be worthwhile, particularly given the concern that both drugs may be metabolized by CYP2C19.1 However, our ability to test this association within our prospective study was limited by the relatively short follow-up period after the use of clopidogrel became routine.

Association Between Proton Pump Inhibitor Use and Risk of  Stroke
We read with interest the article by Nguyen et  al1 evaluating the association between proton pump inhibitor (PPI) use and ischemic stroke. The authors found that PPI use was associated with a significant increase in the risk of ischemic stroke after adjustment for established risk factors for stroke including smoking status, body mass index, p hysical activity, dietary quality, alcohol use, menopausal hormone use in women, multivitamin use, regular aspirin use, regular nonaspirin nonsteroidal anti-inflammatory drug, use and personal history of hypertension, hyperlipidemia, coronary artery disease, or diabetes.

Incidence, Risk Factors, and Clinical Effects of Recurrent Diverticular Hemorrhage: A Large Cohort Study
Although recurrent diverticular hemorrhage is common, its incidence and risk factors have not been measured outside of small institutional cohorts. We analyzed the incidence of and risk factors for recurrent diverticular hemorrhage and whether discontinuing anticoagulation after diverticular hemorrhage is associated with ischemic stroke.

An Unusual Cause of Abdominal Pain in a Young, Hypertensive Female
Question: A 21-year-old woman of Indian descent presented with subacute left upper quadrant abdominal pain. The pain was sharp, severe, and elicited by food. She denied diarrhea, constipation, and weight loss. Her past medical history was significant for hypertension diagnosed as a teenager and ischemic stroke, attributed to Moya-Moya disease, and treated with stenting and bypass of the left middle cerebral and internal carotid arteries. Her medications included amlodipine, aspirin, and topiramate.
