Results After Thoracic Aortic Reoperations in Marfan Syndrome.
CONCLUSIONS: Aortic reoperations in patients with Marfan syndrome can be performed with acceptable midterm outcomes. Performed as a bailout procedure, stent grafting was not associated with increased mortality. The only strong predictor of mortality after reoperations is acute dissection at the time of the initial treatment. PMID: 24518572 [PubMed - as supplied by publisher]

Thromboembolism in a patient with a mechanical mitral valve during anticoagulation with dabigatran etexilate.
We report a 57-year-old man with a mechanical heart valve who experienced acute upper limb thromboembolism during dabigatran intake. Dabigatran might be inadequate for thromboprophylaxis after mechanical valve replacement. PMID: 24182477 [PubMed - in process]

Transapical Aortic Valve Implantation in Patients With Previous Cardiac Surgery.
CONCLUSIONS: Transapical transcatheter aortic valve implantation has simplified surgical treatment of high-risk patients with previous cardiac surgery and severe aortic valve stenosis and is associated with minimal risk of stroke. Furthermore, current data suggest that the presence of previous cardiac surgery does not impair outcomes after transapical transcatheter aortic valve implantation, making this subset of patients particularly applicable for this evolving approach. Sutureless Implantation of the Perceval S Aortic Valve Prosthesis Through Right Anterior Minithoracotomy.
CONCLUSIONS: A sutureless valve for minimally invasive aortic valve replacement is a feasible, effective, and safe tool. Ultimately amplifying indications for less invasive aortic valve replacement in a high surgical risk subset of patients, it can become a valid alternative for transcatheter aortic valve implantation. Minimally Invasive Mitral Valve Surgery: Influence of Aortic Clamping Technique on Early Outcomes.
CONCLUSIONS: Minimally invasive mitral valve surgery can be performed successfully using either the endoaortic balloon technique or the transthoracic clamp approach. Outcome of redo surgical aortic valve replacement in patients ≥ 80 years: results from the multicenter RECORD initiative.
CONCLUSIONS: Octogenarians undergoing S-AVR after prior cardiac surgery have similar immediate postoperative outcome to younger patients and their 5-year outcome is excellent. Transapical Aortic Valve Replacement for Severe Aortic Stenosis: Results From the Nonrandomized Continued Access Cohort of the PARTNER Trial.
CONCLUSIONS: Among the 975 patients in the NRCA-TA cohort, rates of major outcomes including death and stroke compared favorably with outcomes of PMA-TA and SAVR patients enrolled in the PARTNER trial. This trend toward improved outcomes may be attributed to improved patient selection, individual centers surmounting the procedural learning curve, and refinements in surgical technique. Minimally Invasive Fibrillating Heart Surgery: A Safe and Effective Approach for Mitral Valve and Surgical Ablation for Atrial Fibrillation.
CONCLUSIONS: We demonstrated that low operative mortality and low stroke rate with MI fibrillating cardiac operations without cross-clamping allows for MVS and AF ablation. Variation in Warfarin Thromboprophylaxis After Mitral Valve Repair: Does Equipoise Exist and Is a Randomized Trial Warranted?
CONCLUSIONS: At present, half of patients are prescribed warfarin after isolated mitral valve repair in North American cardiac surgical practice which may impact the length of hospital stay. Although patient-level predictors of warfarin prescription exist, center- and surgeon-level variations are prominent.
