Causes and Secondary Prevention of Acute Ischemic Stroke in Adults.
Abstract Stroke still remains a major cause of death and disability worldwide. Ischemic stroke is the most common type of stroke. Causes of ischemic stroke can be classified into large-artery atherosclerosis, cardiogenic embolism, small-vessel disease, stroke of other determined etiology, and stroke of undetermined etiology. Stroke causes in adults are mainly attributable to classical cardiovascular risk factors such as hypertension, diabetes, hypercholesterolemia, and smoking. In neuroimaging, stroke subtypes can be defined according to lesion localization and distribution (territorial infarct, lacunar infarct, h...

Childhood stroke : What are the special features of childhood stroke?
Abstract Childhood arterial ischemic stroke differs in essential aspects from adult stroke. It is rare, often relatively unknown among laypersons and physicians and the wide variety of age-specific differential diagnoses (stroke mimics) as well as less established care structures often lead to a considerable delay in the diagnosis of stroke. The possible treatment options in childhood are mostly off-label. Experiences in well-established acute treatment modalities in adult stroke, such as thrombolysis and mechanical thrombectomy are therefore limited in children and only based on case reports and case series. The...

Childhood Arterial Ischaemic Stroke: Clinical Presentation, Risk Factors and Management.
Abstract Childhood arterial ischaemic stroke (AIS) is a rare, but potentially life-threatening event which requires early diagnosis and adequate treatment. The reported significant time delay to childhood AIS diagnosis may be associated with low awareness, the more nonspecific clinical presentation as well as difficult clinical differentiation to more common "stroke mimics" and a less established "acute care structure" with delayed access to proper neuroimaging. Compared with adult stroke care, experiences with acute reperfusion therapies like thrombolysis and mechanical thrombectomy are promising but limited and ...

Platelets in Thrombo-Inflammation: Concepts, Mechanisms, and Therapeutic Strategies for Ischemic Stroke.
Abstract Platelets are anucleate cells known for their essential function in hemostasis and formation of thrombi under pathologic conditions. In recent years, strong evidence emerged demonstrating the critical involvement of platelets in inflammatory processes including acute ischemic stroke (AIS), which is one of the leading causes of death and disability worldwide. Recanalization of the occluded brain artery to reconstitute cerebral blood flow is the primary goal in the treatment of stroke patients. However, despite successful reperfusion many patients show progression of infarct sizes, a phenomenon referred to ...

Stroke alarm-Arterial ischemic stroke as one of the most time-critical emergencies in children and adolescents
Nervenarzt. 2022 Jan 24:1-9. doi: 10.1007/s00115-021-01252-4. Online ahead of print.ABSTRACTArterial ischemic stroke in childhood and adolescence is one of the most time-critical emergencies in pediatrics. Nevertheless, it is often diagnosed with a considerable time delay which may be associated with low awareness, the sometimes nonspecific clinical presentation with a wide variety of differential diagnoses, and less established 'acute care structures'. The revascularisation strategies in adult stroke care are also potential and promising treatment options for children, even if available evidence is still limited. In the p...

Edoxaban for stroke prevention in atrial fibrillation and treatment of venous thromboembolism: an expert position paper.
Abstract Edoxaban is the most recent available representative of the Non-Vitamin K antagonist oral anticoagulants (NOAC). The approval was based on the largest phase III trials of NOACs for stroke prevention in patients with non-valvular atrial fibrillation (AF, ENGAGE-AF), and for the treatment of venous thromboembolism (VTE, HOKUSAI-VTE). In both trials, edoxaban was associated with similar efficacy and a significant reduction in bleeding events with respect to the pre-defined primary safety endpoints, as compared to warfarin.Additionally, the once daily dosing of edoxaban, the clinically investigated strateg...

Consensus statement: stroke prevention in nonvalvular atrial fibrillation in special consideration of the new direct oral anticoagulants.
–sterreichische Gesellschaft für Hämatologie & Medizinische Onkologie (OeGHO), Österreichische Schlaganfall-Gesellschaft (ÖGSF), Österreichische Gesellschaft für Innere Medizin (ÖGIM), Österreichische Gesellschaft für Anästhesiologie, Reanimation und Intensivmedizin (ÖGARI), Österreichische Gesellschaft für Allgemein- und Familienmedizin (ÖGAM) Abstract The introduction of new direct oral anticoagulants has changed the treatment of nonvalvular atrial fibrillation. However, these changes are not yet fully reflected in current guidelines. This consensus statement, endorsed by six Austrian medical so...

Arterial ischemic stroke in childhood and adolescence
Monatsschr Kinderheilkd. 2022 Apr 20:1-13. doi: 10.1007/s00112-022-01465-0. Online ahead of print.ABSTRACTChildhood arterial ischemic stroke is one of the most time-critical pediatric emergencies but is often diagnosed with a prognostically relevant time delay. The reasons are low awareness, sometimes unspecific clinical presentation with a wide variety of critical differential diagnoses and less coordinated acute care structures. The revascularization strategies established for adults also show sometimes spectacular success in children. These should therefore also be made available for affected children if possible, altho...
