E-061 Glycoprotein inhibitors as a standalone post-thrombectomy rescue treatment for intracranial atherosclerotic disease-related stroke: a systematic review and meta-analysis
ConclusionThese findings highlight the potential value of GPIs as a viable first line therapy for EVT rescue therapy in the setting of ICAD. This study was limited by the nature of the literature, which were non-randomized observational studies. Randomized controlled trials are needed to validate and refine our understanding of the role of GPIs in the management of refractory or re-occlusive ICAD stroke.Disclosures A. Brake: None. C. P005/44 Using the pEGASUS-stent with antithrombogenic properties and single antiplatelet therapy in a case of thrombectomy
IntroductionRescue stenting is used more and more in stroke cases with large-vessel-occlusion or medium-vessel-occlusion refractory to mechanical thrombectomy. The pEGASUS-stent(Phenox) represents a device which is equipped with a hydrophilic polymer coating(HPC) with antithrombogenic properties. The coating allows for implantation under single antiplatelet therapy, thus possibly reducing the risk of bleeding in acute stroke cases.Case HistoryA 79-year-old woman was rushed to emergency room due to left-sided hemiparesis and National-Institutes-of-Health-Stroke-Scale(NIHSS) of 14. Computed-tomography-image showed an occlusi...

Can Renal Denervation Reduce Heart Attack, Stroke and Death?
Renal denervation, a catheter-based procedure designed to lower blood pressure, has been the subject of much interest to the interventional community over the past couple of years. Although high blood pressure puts one in three adults worldwide at risk of heart attack, stroke and kidney failure, the renal denervation studies that have been conducted to date have looked only at reducing blood pressure rather than the overall impact on major cardiac events. St. Jude's (NYSE:STJ) announcement today of the EnligHTNment trial is the first that will look at "hard end points" such as heart attack, stroke and mortality.

Review of an article: Protamine does not increase risk of stroke in patients with elective carotid stenting by Jennifer S. McDonald, PhD; David F. Kallmes, MD; Giuseppe Lanzino, MD; Harry J. Cloft, MD, PhD (Stroke 2013;44:2028-30)
The use of the heparin antagonist protamine after percutaneous intervention for carotid stenosis has been a topic of discussion in the literature as the safety of protamine after carotid angioplasty and stenting procedures has been called into question. Protamine sulfate is a cationic protein that binds with heparin to reverse the anticoagulant activity of unfractionated heparin. It is administered to reduce access site bleeding complications after femoral sheath removal, but its use must be balanced with the standard preprocedural anticoagulation undertaken to prevent thromboembolic complications. The purpose of this stu...

Comment Improved medical treatment in secondary prevention of stroke
Atherosclerotic intracranial arterial stenosis is a cause of transient ischaemic attack and ischaemic stroke, and is associated with a high risk of recurrence. Patients with recently symptomatic severe (70–99%) stenosis are at particularly high risk. In the SAMMPRIS trial, investigators compared two alternative treatment strategies in patients with a recent (<30 days) transient ischaemic attack or non-disabling stroke attributed to angiographically verified 70–99% intracranial stenosis: aggressive medical treatment (combination antiplatelet therapy and intensive management of risk factors) versus aggressive medical ...

Acute Stroke Intervention
Abstract: Ischemic strokes will make up most (>80%) of the three-quarters of a million strokes that will occur in Americans this year. Reperfusion therapy is the fundamental strategy for the treatment of acute ischemic stroke. Reperfusion therapy may be accomplished noninvasively (intravenous thrombolysis) or invasively with catheter-based treatments (intra-arterial thrombolysis, thrombectomy, or angioplasty). Currently, a large majority of patients with acute ischemic stroke do not receive any form of reperfusion therapy owing to their delayed presentation (>3 hours) and lack of skilled man power for on-demand endovascula...

Simultaneous onset of myocardial infarction and ischemic stroke in a patient with atrial fibrillation: Multiple territory injury revealed on angiography and magnetic resonance
An 84-year-old man with a history of hypertension and paroxysmal atrial fibrillation (AF) who received no anticoagulant drugs experienced acute chest pain and transient loss of consciousness. He was transferred to our emergency room. His initial electrocardiogram showed sinus rhythm with ST-segment elevation in the I, aVL, and V1–V6 leads. His blood pressure was 158/92mmHg and his pulse was regular at 70beats per minute. A chest radiograph showed increased heart size and pulmonary vascular congestion. His troponin T level was elevated (more than 0.1ng/mL). He was diagnosed with ST-elevation myocardial infarction (MI) and...

Carotid artery stenting in nonagenarians: are there benefits in surgically treating this high risk population?
Conclusions CAS in nonagenarian patients carried increased risk of perioperative ischemic events, compared with contemporary trial results in symptomatic and asymptomatic patients. Further, mean survival time postprocedure fell short of guidelines for receiving procedural benefit. Although larger scale multicenter research is needed, we recommend careful consideration of overall health status when contemplating stenting in nonagenarians.
