Race/Ethnic Disparities for In-Hospital Mortality and Disability at Discharge after Acute Ischemic Stroke: Florida Puerto Rico Collaboration to Reduce Stroke Disparities (P5.287)
Conclusions:In our large Florida Stroke Registry, we observed lower in-hospital mortality, but greater disability at discharge after stroke among blacks in comparison to whites. This paradox needs further exploration to design and implement interventions to improve stroke outcome and reduce disparities in post-stroke disability.Disclosure: Dr. Simonetto has nothing to disclose. Increased risk of stroke in patients with chronic kidney disease after recurrent hypoglycemia
Conclusions: CKD was associated with a higher risk of stroke and mortality in patients with hypoglycemia. Recurrent hypoglycemia considerably increased the risk of stroke and overall mortality in patients with CKD regardless of whether they had diabetes. These results suggest that hypoglycemia has a crucial role in stroke and death in patients with CKD.

The Impact of a Stroke Prevention Clinic after Discharge for TIA/ Stroke: University Hospitals Case Medical Center Comprehensive Stroke Center Experience (P3.121)
Conclusions: The burden of risk factors in our patients was very high. Although patients reported uniform adherence to treatment, risk factor control to target was good for hypertension and atrial fibrillation and poor for smoking, diabetes and dyslipidemia. Despite high rates of insurance and established primary care, multiple interventions were performed during this visit, particularly in the more disabled. The post-discharge SPC visit augments primary care and offers an important opportunity to review and revise the care plan, assess patient adherence and control of risk factors to target.*Chart and Graph imaging to be ...

Racial differences in statin adherence following hospital discharge for ischemic stroke
Conclusions: Compared with white adults, black adults were more likely to be nonadherent to statins following hospitalization for ischemic stroke.

Sex Disparities in Discharge Ambulation Status after Acute Ischemic Stroke: The Florida-Puerto Rico Collaboration to Reduce Stroke Disparities (FL-PR CReSD) (P5.145)
Conclusions:Only half of AIS survivors were able to ambulate independently after stroke at discharge. Independent ambulation at discharge was less likely achieved in women than men, particularly among patients who were able to independently ambulate before stroke. Future studies should examine the underlying causes of worse ambulation outcomes in women.Disclosure: Dr. Oluwole has nothing to disclose. Dr. Wang has nothing to disclose. The First 3 Months Post Stroke: A Unique Opportunity to Promote Exercise (P3.300)
Conclusions: Ischemic stroke patients were more likely to be exercising at three months than before their stroke, likely in part due to prescribed rehabilitation. Although this analysis is limited by unknown aerobic intensity, this subacute time period after stroke may provide a unique opportunity to incorporate exercise into patients’ lifestyles long-term.Disclosure: Dr. Garrison has nothing to disclose. Diabetic microvascular complications and stroke risk in the Asian population in Taiwan: A population based case-control study (P6.290)
Conclusions:This study suggests that microvascular complications are significant stroke predictors, with a greater involvement for ischemic stroke than for hemorrhagic stroke. Multiple microvascular complications interactively increase the stroke risk. Our study contributes to the identification of high-risk subjects for stroke prevention and adequate glycemic control.Disclosure: Dr. Chen has nothing to disclose. Dr. Lo has nothing to disclose. Conclusion: Even in these selected population, with previous or current CVD, a large amount of RF and a high rate of previous medical consultations, knowledge about stroke showed to be very low indicating a poor quality of preventive measures and the need to emphasize educational interventions.Disclosure: Dr. Muñoz has nothing to disclose. Hospital costs of ischemic stroke and TIA in the Netherlands
Conclusions: Hospital costs are higher for inpatients and ischemic strokes compared with outpatients and TIAs, with length of stay (LOS) the most important contributor. LOS and hospital costs have substantially declined over the last 10 years, possibly due to improved hospital stroke care and efficient integrated stroke services.

A Decade of Racial and Ethnic Stroke Disparity: An Insight from Nhanes (P03.151)
CONCLUSIONS: The greater prevalence of stroke in AA compared to NHW is better explained by variables that reflect the diagnosis and control of important vascular risk factors. These results identify targets for interventions to reduce this disparity.Disclosure: Dr. Gutierrez has nothing to disclose.

CARES (Changing and Advancing Risk factor control through Educations after Stroke): A Pilot Trial of a Transitions in Care Post-discharge Telephone Intervention for Stroke Patients (S13.006)
OBJECTIVE: The purpose of study is to test the feasibility and effectiveness of a telephonic post-discharge intervention designed to improve access to care, reinforce self-management skills, improve health literacy, and screen for complications in a government-run safety net system. BACKGROUND: In the United States, risk factor control after stroke is poor, particularly among socio-economically disadvantaged populations with poor access to care. The immediate post-discharge period is a vulnerable time for stroke patients, given their risk of recurrent stroke, worsening symptoms, complications, and need for medication titra...
