E-029 Thrombectomy in stroke patients with low ASPECTS: is TICI 2C/3 superior to TICI 2B?
ConclusionIn patients with low ASPECTS, achieving a TICI 2C/3 score after MT is associated with a more favorable 90-day outcome. These findings suggest that TICI 2C/3 is a better target for MT than TICI 2B in patients with low ASPECTS.Abstract E-029 Figure 1Disclosures S. Samir Elawady: None. M. Mahdi Sowlat: None. I. E-108 Endovascular treatment of patients with acute ischemic stroke due to underlying arterial dissection: a nationwide analysis
ConclusionsDespite a similar prevalence among patients admitted for AIS, CA dissections are associated with a higher rate of endovascular intervention and poorer short-term outcomes compared to VA dissections. This study provides essential epidemiologic and outcome data for the cerebrovascular neurosurgeon.Abstract E-108 Table 1Comparison of clinical characteristics and outcomes among patients presenting with acute ischemic stroke due to carotid or vertebral artery dissections Characteristic Vertebral Artery Dissection, N = 2,562 Carotid Artery Dissection, N = 2,789 p-value1 Age (years), Mean (SD) 53 (17) 57 (17) <0.00...

E-145 Mortality following mechanical thrombectomy for ischemic stroke in patients with COVID-19
ConclusionOverall, this study indicates that COVID-19 is a predictor of mortality among mechanical thrombectomy. This finding is likely multifactorial but may be related to multisystem inflammation, hypercoagulability, and re-occlusion seen in COVID-19 patients. Treatment-related factors, such as delays in care, may also play a role. Additional research would be needed to further delineate these findings.Disclosures J. Dallas: None. T. Wenger: None. K. E-247 The universal effect of the 'golden hour on stroke thrombectomy outcomes
ConclusionsIn this work we demonstrate the universality of the ‘Golden Hour’ effect, in which procedures lasting more than 1 hour are associated with worse clinical outcomes and higher rates of symptomatic hemorrhage. This study will assist clinicians in determining when to be more aggressive in their approach when aiming for TICI 3 reperfusion in different subpopulations of patients.Disclosures M. Ash: None. L. P175/308 Analysis of demographic, procedural and imaging data of patients with acute arteriosclerotic tandem lesion: Age and stroke severity matters most for clinical outcome
ConclusionEVT for TL is safe and effective. Lower age and shorter procedural time both correlate with successful outcomes, but the fact that stroke severity, neurological issues and diabetes are linked to higher mortality shows the connotation of comorbidities with long-term survival.Disclosure of InterestAll authors: Nothing to disclose

O-008 The Financial Impact of ADAPT for Endovascular Treatment of Acute Ischemic Stroke
Conclusion The most cost effective approach to a large intracranial vessel occlusion appears to be direct aspiration with a large bore catheter (without the separator) first and if this fails then proceed with other devices such as a stentriever. P-031 Single Center Experience with the ADAPT Technique for Acute Ischemic Stroke
We report our single center experience of endovascular treatment of stroke by a technique of direct aspiration with large bore catheter. Methods From September 2013 to March 2014 we included prospectively on intent to treat 80 patients presenting with large vessels occlusion in the settings of acute ischemic stroke. The ADAPT technique was utilised in first intention in all patients and was associated with stentriever technique when necessary. Procedural and clinical data were collected for analysis. Results Patients presented with an admitting median National Institutes of Health Stroke Scale (NIHSS) score of 15 (1&ndas...

A research roadmap of future endovascular stroke trials
The recent completion of the MR CLEAN trial1 and news of early stoppage of other stroke trials demonstrates the ability for the neurointerventional community to address a crucial question that has hindered the ability of intra-arterial therapy (IAT) to be offered more widely. The focus of future studies will now shift towards improving clinical outcomes in patients undergoing IAT. Patient selectionImaging There is currently no consensus regarding the optimal imaging strategy for the selection of patients for intervention. The modality must be efficient, accurate, available and repeatable. Non-contrast CT using Alberta Stro...

Unprotected carotid artery stenting in symptomatic elderly patients: a single-center experience
Conclusions Endovascular treatment of carotid stenosis without protection devices in symptomatic patients aged >75 years is an alternative to endovascular treatment with protection devices. Complications and mortality rates are similar to studies that used protection devices in lower risk patients.

Watching, but not waiting: vascular neurology perspective on the disparate regulatory pathways for stroke
Vascular neurologists have keenly watched the Watchman device (Atritech, Plymouth, Minnesota, USA) regulatory approval process. We are, as always, searching for additional options in the prevention and treatment of stroke to better care for our patients, and new approaches to the management of atrial fibrillation play a large part in this effort. Recently, a Food and Drug Administration (FDA) panel voted 13:1 in favor of the Watchman device for the prevention of ischemic stroke in non-valvular atrial fibrillation.1 The panelists came to this decision after reviewing data from large randomized trials that compared anticoagu...

E-006 the time of recanalization since symptoms is a predictor of outcome in patients who underwent stent retriever thrombectomy for acute ischemic stroke from middle cerebral artery occlusion
ConclusionsOur study demonstrates that patients with LAO from MCA who recanalize less than 3 h of symptoms onset have very good chance of good outcome (0–2) compared to those who recanilize more than 3 h. Therefore, all AIS patients with LAO should offer early SRT to achieve a good functional outcome. Further studies are required.DisclosuresY. Lodi: None. V. Reddy: None. A. Devasenapathy: None. A.
