Resource use and quality of care in an urban stroke unit
Conclusion Better outcome measures in ASU could be attributed to the differences between the groups of stroke patients. Prompt therapist input and frequent nursing observations have long been considered to be imperative in the success of ASU; our findings wholly support this and highlight the importance of the multidisciplinary approach. The snapshot data revealed that bed capacity on ASU was sufficient for the number of stroke patients. However capacity was not being utilised efficiently as non–stroke medical boarders occupied beds consistently whilst stroke patients were boarded out to other wards.

Poststroke dementia is associated with recurrent ischaemic stroke
Conclusions Poststroke dementia predicts recurrence of ischaemic stroke in long-term follow-up and should be considered when estimating prognosis.

Rates and predictors of risk of stroke and its subtypes in diabetes: a prospective observational study
Conclusions Older people with a history of stroke were at highest risk of stroke, but the prognosis and prognostic factors of subtypes were heterogeneous. The results will help clinicians quantify the absolute risk of stroke and its subtypes for typical diabetes patients.

Conclusions Impaired perfusion modified the relationship between blood pressure and stroke risk, although this study had limitations including the retrospective analysis, the potentially biased sample, the small number of critical events and the fact that BP was measured only as a snapshot in clinic.

Association of obstructive sleep apnoea (osa) with incident stroke: a systematic review and meta-analysis
Conclusion As OSA appears to be independently associated with stroke (and continuous positive airway pressure (CPAP) reduces such cardiovascular outcomes in OSA6), clinicians should look out for the symptoms in stroke patients so appropriate risk mitigation strategies may be implemented. Equally, patients with OSA but no history of stroke should receive appropriate management to lower their cardiovascular risk, be it through treatment of OSA with CPAP or modification of traditional risk factors such as obesity and hypertension.

Stress resilience in male adolescents and subsequent stroke risk: cohort study
Conclusions Stress susceptibility and, therefore, psychosocial stress may be implicated in the aetiology of stroke. This association may be explained, in part, by poorer physical fitness. Effective prevention might focus on behaviour/lifestyle and psychosocial stress.

Conclusions Blood pressure elevation persists throughout ECP treatment, which consists of 35 sessions. However, cerebral blood flow augmentation may last at least 3 weeks and then appears to return to baseline 1 month after acute stroke onset.

Mortality following stroke during and after acute care according to neighbourhood deprivation: a disease registry study
Conclusions The gradient of higher poststroke mortality with increasing neighbourhood deprivation was noticeable only after acute hospital discharge. Quality of postacute care and social support are potential determinants of these variations.

Identification of missed hypertension and hypertensive arteriopathy with home versus ambulatory blood pressure measurement in patients with tia or minor stroke
Conclusions In patients with TIA or minor stroke, HBPM was more reliable than ABPM at identifying missed hypertension and hypertensive arteriopathy. Its use could significantly reduce the risk of recurrent stroke.

Conclusions TCD with bubble test is sensitive and specific for the detection of right–to–left shunts due to PFO confirmed at right heart catheterisation. In this population, TOE had low sensitivity for RLS. There was no difference in physical PFO size between TOE positive and negative patients.

Is the outpatient management of acute minor stroke feasible and safe?
Paul et al1 have assessed the clinical outcomes, early hospital admission rates and hospital care costs in clinic referred and hospital referred minor stroke patients in a prospective population based study. They did not find significant differences in the 30 day admission rate in clinic patients compared with the 30 day readmission rate after discharge in hospital treated patients (16/237 vs 9/150). Conclusions PVS do not have an independent association with cognitive impairment in patients with ischaemic stroke or TIA. The associations with clinical-radiological factors are consistent with the hypothesis that PVS reflect cerebral small vessel disease; the different associations for basal ganglia and centrum semiovale PVS might indicate different underlying small vessel arteriopathies according to PVS anatomical distribution, but this requires further study.

Predictors of futile recanalization after endovascular treatment in acute ischemic stroke: a meta-analysis
Conclusion This study identified female sex, comorbidities, admission systolic blood pressure, serum glucose, occlusion site, non-bridging therapy, and post-procedural complication as predictors of futile recanalization, and also confirmed previously reported factors. Further large-scale prospective studies are needed.
