Silent Myocardial Infarction: A Case Report
We present a case of a 59-year-old overweight woman with prediabetes, primary hypertension, and hypercholesterolemia who presented for herpes zoster (HZ) follow-up; she reported having skipped heartbeats and heart rate fluctuations during the review of systems. On further workup, ECG revealed low voltage QRS complexes, flat QRS complexes, flat T waves, and pathological Q waves, suggesting the diagnosis of SMI. Based on the identified risk factors, including high BMI, prediabetes, primary hypertension, hypercholesterolemia, HZ, and newly diagnosed SMI, the patient was advised to continue with lisinopril 20 mg daily, prescri...

Impact of influenza, herpes zoster, and pneumococcal vaccinations on the incidence of cardiovascular events in subjects aged over 65  years: a systematic review
Geroscience. 2023 Jun 3. doi: 10.1007/s11357-023-00807-4. Online ahead of print.ABSTRACTThis systematic review aims to summarize the impact of vaccination against influenza, shingles, and pneumococcus on the incidence on the risk of cardiovascular events in the elderly. This protocol was developed in accordance with PRISMA guidelines. We conducted a literature search and identified all relevant articles published regarding the matter up to September 2022. We retrieved 38 studies (influenza vaccine = 33, pneumococcal vaccine = 5, and zoster vaccine = 2). A total of 28 and 2 studies have shown that influenza and pneumococcal...

Effects of herpes zoster vaccination and antiviral treatment on the risk of stroke: a systematic review and meta-analysis
CONCLUSIONS: This meta-analysis suggests that the HZV, but not antiviral treatment, decreases the odds of developing stroke.PMID:37265460 | PMC:PMC10231675 | DOI:10.3389/fneur.2023.1176920

Trend in the Numbers of Hospitalized Patients With Varicella, Herpes Zoster, and Ischemic Stroke in Japanese Individuals & lt;20 Years of Age Before and After Implementation of Universal Varicella Vaccination
Pediatr Infect Dis J. 2023 Mar 20. doi: 10.1097/INF.0000000000003907. Online ahead of print.ABSTRACTSeveral studies have shown an association between varicella-zoster virus infection and ischemic stroke. We analyzed the trends in the numbers of patients with varicella, herpes zoster and ischemic stroke before and after the universal vaccination program using a Japanese database of hospitalized patients. The number of patients with varicella decreased but those of herpes zoster and ischemic stroke did not change.PMID:36996309 | DOI:10.1097/INF.0000000000003907

Herpes zoster and long-term vascular risk: a retrospective cohort study
Sci Rep. 2023 Feb 9;13(1):2364. doi: 10.1038/s41598-023-29667-w.ABSTRACTHerpes zoster (HZ) represents a serious health problem in the general population due to its abundance and complications. Stroke and acute myocardial infarction are well-documented short-term complications of HZ, primarily due to vasculopathy in the cerebral and coronary arteries. However, no major study to date has specifically demonstrated that HZ is a long-term risk factor for all Major Adverse Cardiac and Cerebrovascular Events (MACCE). A retrospective cohort study was conducted analyzing the association between HZ and MACCE. We compared HZ patients...

Predictors of herpes zoster vaccination among Australian adults aged 65 and over
CONCLUSION(S): Further research is required to understand the barriers to HZ vaccine uptake. Increasing the funding eligibility for those who are at risk of complications from shingles, or lowering the age of eligibility, may increase vaccine coverage.PMID:36336528 | DOI:10.1016/j.vaccine.2022.10.064

Zoster-associated Prothrombotic Plasma Exosomes and Increased Stroke Risk
J Infect Dis. 2022 Oct 6:jiac405. doi: 10.1093/infdis/jiac405. Online ahead of print.ABSTRACTHerpes zoster (HZ, shingles) caused by varicella zoster virus (VZV) reactivation increases stroke risk for up to one-year post-HZ. The underlying mechanisms are unclear, however, the development of stroke distant from the site of zoster (e.g. thoracic, lumbar, sacral) that can occur months after resolution of rash points to a long-lasting, virus-induced soluble factor(s) that can trigger thrombosis and/or vasculitis. Herein, we investigated the content and contributions of circulating plasma exosomes from HZ and non-HZ patient samp...

Active Post-Licensure Safety Surveillance for Recombinant Zoster Vaccine Using Electronic Health Record Data
Am J Epidemiol. 2022 Oct 4:kwac170. doi: 10.1093/aje/kwac170. Online ahead of print.ABSTRACTRecombinant zoster vaccine (RZV) (Shingrix; GlaxoSmithKline, Brentford, United Kingdom) is an adjuvanted glycoprotein vaccine that was licensed in 2017 to prevent herpes zoster and its complications in older adults. In this prospective, post-licensure Vaccine Safety Datalink (VSD) study using electronic health records, we sequentially monitored a real-world population of adults aged 50 years and older who received care at multiple VSD health systems in the United States to identify potential increased risks of 10 pre-specified prior...

Rare Adverse Events Associated with BNT162b2 mRNA Vaccine (Pfizer-BioNTech): A Review of Large-Scale, Controlled Surveillance Studies
In conclusion, this review has concisely summarized the current rare adverse events related and unrelated to BNT162b2, arguably for the first time in sufficient depth, to better communicate vaccine safety to the public.PMID:35891231 | DOI:10.3390/vaccines10071067

Increased Stroke Risk Following Herpes Zoster Infection and Protection with Zoster Vaccine
CONCLUSION: Patients had a significantly higher risk of stroke within the first month following recent herpes zoster infection. Receipt of at least one zoster vaccination was found to mitigate this increased risk. Vaccination may therefore be viewed as a protective tool against the risk of neurologic post-infection sequelae.PMID:35796546 | DOI:10.1093/cid/ciac549

Comparative Safety of BNT162b2 and mRNA-1273 Vaccines in a Nationwide Cohort of US Veterans
CONCLUSIONS AND RELEVANCE: The findings of this cohort study suggest that there were few differences in risk of adverse events within 14 days of the first dose of either the BNT162b2 or the mRNA-1273 vaccine and small-magnitude differences within 42 days of the first dose. The 38-week risks of adverse events were low in both vaccine groups, although risks were lower for recipients of the mRNA-1273 vaccine than for recipients of the BNT162b2 vaccine. Although the primary analysis was designed to detect safety events unrelated to SARS-CoV-2 infection, the possibility that these differences may partially be explained by a low...
