E-104 Young man with osteogenesis imperfecta presenting with acute stroke from carotid dissection and subsequent contralateral pseudoaneurysm
ConclusionOsteogenesis imperfecta (OI) has been associated with numerous cerebrovascular conditions including carotid cavernous fistulas, cerebral aneurysms, and carotid dissections with attendant risk of stroke. While our patient was able to be successfully treated for his initial carotid dissection in the acute phase, he was at higher risk for stroke, whereas his subsequent carotid dissection was identified pre-symptomatically and treated electively. Judicious evaluation of patients with OI for cervical carotid dissections is warranted for early identification and treatment.Disclosures A. Mbaluka: None. R. Sanchez-Mejia:...

E-131 Transfemoral versus transradial approach for cerebral angiography: A patient preference survey
ConclusionsAt our institution, most patients preferred transradial over transfemoral approach. Most common reasons included pain, recovery time and bruising. Although the effectiveness and safety of transradial approach for non-diagnostic neurointervention remains uncertain and continues to technically evolve, it may be the best option for diagnostic angiograms considering the patients’ preference.Disclosures J. E-141 CT angiography in patients presenting with lower NIH stroke scales
ConclusionCTP should be ordered for patients presenting with strokes with NIH < 6. Also, Boned et al. showed that CTP overestimated infarct core for more than 10 mL in 38% of the patients. Choosing not to proceed with CT angiography based on CTP and NIH stroke scale-based patient selection may deny treatment to patients who might benefit from reperfusion therapy. 71% of M2 strokes present with Tmax > 6 of 44ml which could potentially be considered for endovascular treatments.Disclosures M. E-145 Safety of pediatric cerebral angiography
ConclusionsPediatric cerebral angiography has a low rate of major or permanent complications. Children with history of hypertension, history of ischemic stroke, or female sex are at higher risk of complications, while use of femoral access carries lower risk of complications.Disclosures D. E-073 Clot bank collaborative registry protocol: Novel method for evaluating ischemic thrombus
ConclusionWe have developed a novel tissue banking protocol to preserve microbiota from arterial thrombi retrieved from ischemic stroke patients. While there is no consensus which class of pathogens is implicated in the susceptibility to stroke, multiple studies including our own, have shown bacterial fragments and communities in cerebral thrombi. The specimens captured during standard-of-care thrombectomy allow us to characterize and functionally define the microbiota associated with cerebral thrombi in ischemic stroke patients.Disclosures M. E-080 Switching from transfemoral to transradial access during neuroendovascular procedures: A single-center retrospective review of outcome and complications
ConclusionIn the experience of this single-center, the use of the transradial approach for neuroendovascular procedures seems to result in a relatively low rate of access complications and shifting access technique. Our results, while limited in the size of sample, contribute to the growing body of literature that sheds insight on the benefit of transradial access.Disclosures M. Taqi: None.

E-088 Visualization of the anterior temporal artery as a predictor of outcome after mechanical thrombectomy
ConclusionThough the presence of the anterior temporal artery in patients undergoing recanalization for MCA occlusion did not reach statistical significance for predicting independent outcomes, there was a strong trend for improved outcomes. Particularly for institutions without CTP capability or when CTP data is not-reliable, this association with ATA visualization should be further investigated as a predictor for good outcome after successful mechanical thrombectomy.Disclosures J. E-093 Acute ischemic stroke from fibrocartilaginous embolism to the basilar artery
DiscussionIn review of the CTA of the neck, there is a prominent lateral disc protrusion at C5-C6 with a filling defect in the adjacent medial VA. An MRI c-spine confirmed a lateral disc protrusion at this level. In combination with a lateral disc osteophyte, this may have led to medial disruption of the VA and subsequent herniation of fibrocartilage into the left VA leading to the patient’s embolism. A concordant fine linear filling defect was seen at C5-C6 on cervical angiography. Only one case of FCE resulting in cerebral ischemia exists in the literature. In that case, a 17-year-old female developed a right cereb...

Vertebrobasilar atherosclerotic disease: is it time to revisit angioplasty?
Atherosclerotic occlusive disease of the vertebrobasilar (VB) system is an important cause of posterior circulation ischemic stroke, responsible for approximately one-third of cases,1 and associated with high morbidity. Despite medical therapy, the risk of recurrent stroke is significant, particularly for intracranial VB disease, ranging from 10% to 15% per year.2 The role of hemodynamic insufficiency in the etiology of stroke from VB stenosis/occlusion has long been purported, but until recently, there have been few data to directly support this mechanism. Traditional tissue-based imaging techniques for evaluation of hemo...

Extending the indications for transvenous approach embolization for superficial brain arteriovenous malformations
Conclusions The curative transvenous embolization of superficial intracranial AVMs is feasible and appears safe and effective when strict anatomical selection is respected. This technique extends the current indications for transvenous embolization of intracranial AVMs and may improve cure rates while reducing embolization-related complications.
