E-242 Successful mechanical thrombectomy through a frozen elephant aortic arch graft via radial approach
We report the first published case of a mechanical thrombectomy for acute ischemic stroke via radial access in a patient with status post recent frozen elephant trunk graft (FET) for aortic arch repair. A 69 year-old male presented with a left middle cerebral artery large vessel occlusion two weeks after FET graft repair. After consultation with the patient’s cardiothoracic surgeon, a radial approach was deemed safest to access the left-sided large vessel occlusion. E-108 Ruptured intracranial aneurysm presenting as isolated acute subdural hemorrhage
ConclusionIn rare cases, ruptured intracranial aneurysms can be associated with isolated subdural hemorrhage. Common treatment options include endovascular coiling and microsurgical clipping. However, endovascular repair is often preferred especially when the patient may not be able to tolerate a surgical procedure, as was the case with this patient. In this case, the patient presented with stroke-like symptoms and was found to have a subdural hemorrhage. E-124 Timing surgery and hemorrhagic complications in endocarditis with concomitant cerebral complications
ConclusionsPatients with radiographic evidence of ischemic stroke from septic emboli can safely undergo valvular surgery for IE without increased risk of symptomatic hemorrhage. Stenting the carotid artery from radial access using a Simmons guide catheter
Conclusion Transradial carotid stenting using the described lower profile technique provides another effective option in the array of surgical procedures for the treatment of carotid artery stenosis. Relative procedural and fluoroscopy times may initially be longer compared with transfemoral carotid stenting for experienced CAS operators, although absolute differences are small.

P-059 Race as a social determinant of poor outcomes following unruptured aneurysm surgery
ConclusionIn this nationwide analysis, racial disparities were present in that Black patients were at increased odds of poor outcomes and both Black and Hispanic patients experience longer lengths of stay when compared to White patients after adjusting for several possible confounding factors. In the shared effort to provide equitable care, an important first step is analyzing and acknowledging the differences in objective outcomes as racial disparities in neurosurgery. E-080 Switching from transfemoral to transradial access during neuroendovascular procedures: A single-center retrospective review of outcome and complications
ConclusionIn the experience of this single-center, the use of the transradial approach for neuroendovascular procedures seems to result in a relatively low rate of access complications and shifting access technique. Safety and effectiveness of microsurgical clipping, endovascular coiling, and stent assisted coiling for unruptured anterior communicating artery aneurysms: a systematic analysis of observational studies
Conclusions While there are limitations to the data, EC resulted in a more favorable clinical outcome, and MC resulted in more robust aneurysm repair, for unruptured ACoA aneurysms. SAC had a higher treatment morbidity risk than EC, without reduction in retreatment rate. All treatments were effective in preventing SAH. The current pooled analysis of treatment outcomes provides a useful aid to pretreatment clinical decision making.

Endovascular thrombectomy in the setting of aortic dissection
Conclusions These three cases suggest that endovascular thrombectomy is feasible and can be done safely and efficaciously in patients with aortic dissections and those with Marfan syndrome, although the risks and benefits should be considered as part of any decision-making process. Given that endovascular therapy for acute stroke is now in many situations part of standard care, further studies will be necessary to delineate more precise inclusion and exclusion criteria.

ConclusionsEndovascular repair of the MCA aneurysm inclosing those with wide neck and complex are not only safe and feasible, but associated with high long-term functional outcome. Use of flow-diverting stents as salvage treatment following failed stent-assisted embolization of intracranial aneurysms
We report our experience using flow-diverting stents in the repair of 25 aneurysms for which stent-assisted embolization had failed. Nineteen (76%) of these aneurysms at the 12-month follow-up showed improved Raymond class occlusion, with 38% being completely occluded, and all aneurysms demonstrated decreased filling. One patient developed a moderate permanent neurologic deficit. Appropriate stent sizing, proximal and distal construct coverage, and preventing flow diverter deployment between the previously deployed stent struts are important considerations to ensure wall apposition and prevention of endoleak. Groin complications in endovascular mechanical thrombectomy for acute ischemic stroke: a 10-year single center experience
Conclusions These findings suggest that concerns for groin complications should not preclude the use of BGCs and large-bore sheaths in mechanical thrombectomy for acute ischemic stroke.

Incidence and morbidity of craniocervical arterial dissections in atraumatic subarachnoid hemorrhage patients who underwent aneurysmal repair
Conclusions Our study indicates an annual increase in the incidence of CCADs in patients admitted with SAH who require aneurysmal repair. More than two-thirds of these patients that developed CCADs had undergone endovascular coiling repair. A diagnosis of CCAD increased the length of hospital stay but had no statistically significant association with mortality in this patient population.
