Correlation between cerebral blood volume values and outcomes in endovascular therapy for acute ischemic stroke
Conclusions We found that cortical CBV loss is predictive of poor clinical outcomes, whereas basal ganglia CBV loss is predictive of hemorrhagic transformation but without translation into poor clinical outcomes. Our study findings support published results of baseline preintervention CBV as a predictor of outcomes in patients undergoing intra-arterial stroke therapies.

Conclusions Rates of good outcomes, parenchymal hematoma, and final infarct volumes following endovascular treatment may not be different in patients with WUS compared with patients with witnessed onset of symptoms beyond 8 h.

Collateral flow as causative of good outcomes in endovascular stroke therapy
Conclusions Improved collateral flow in patients with AIS undergoing endovascular therapy was associated with improved radiographic and clinical outcomes. Independent of age, vessel occlusion and time, in patients with comparable ischemic burdens, changes in collateral grade alone led to significant differences in initial stroke severity as well as ultimate clinical outcome.

Operator assessment versus core laboratory adjudication of recanalization following endovascular treatment of acute ischemic stroke: a systematic review and meta-analysis
Conclusion Operator angiographic reads are statistically significantly higher than core laboratory-read readings following stroke thrombectomy, especially for complete recanalization. These differences should be considered when interpreting reports of angiographic outcomes after thrombectomy.

Prospective studies, or a meta-analysis of sub-groups in the recently published prospective trials are needed to confirm the association between those factors, or others, and functional outcome. O-016 Outcomes after endovascular mechanical thrombectomy for low national institutes of health stroke scale (NIHSS): a multicenter study
ConclusionsMT is an effective and safe treatment for appropriately selected ischemic stroke patients presenting with low NIHSS. Diabetes and prior stroke are predictors of functional dependence at 90 days, and prior stroke is also associated with mortality, suggesting that MT should be offered judiciously to patients with these risk factors. E-189 Outcomes of tenecteplase versus alteplase after mechanical thrombectomy in a multiethnic stroke consortium registry
ConclusionIn this prospective, multi-center stroke consortium study, IV TNK in comparison to tPA prior to MT for ischemic stroke resulted in non-inferior outcomes related to angiographic reperfusion and functional status at discharge. These findings compliment the current literature and include a large Hispanic US cohort. Conclusions Solitaire retrieval device achieves a very high rate of recanalazitation in acute ischemic stroke with MCA occlusion and results in a good clinical outcome. However, a considerable valuable time is still lost from ER arrival to IR arrival. Patients who required endotracheal intubation at presentation and or had NIHSS 20 or higher are associated with poor outcome despite TICI3 recanalization. Further studied are required. Disclosures Y. E-096 COVID status is related to clot burden during thrombectomy in acute stroke patients
ConclusionOf 230 patients included in this interim analysis across 24 centers in the United States, between 02/2021 and 01/2022, 16 patients had a history of COVID. Comparing COVID-positive to non-positive, patients with COVID were younger (median 66.5 years vs 71.5 years, p 0.042), and had more intracranial clot by weight (median 101.0 mg vs 42.0 mg, p 0.047). There was a trend toward COVID positive patients to have more clot volume (median 180 cc vs 77.5 cc p 0.08), suggesting the difference was a higher clot burden instead of more density to the thrombus. Association of clot burden score with radiographic and clinical outcomes following Solitaire stent retriever thrombectomy: analysis of the SWIFT PRIME trial
Conclusions The combination of IV thrombolysis and stent retriever thrombectomy with the Solitaire device is highly effective in achieving successful recanalization and a good clinical outcome throughout the entire range of CBS values.

Conclusions Although our initial data confirm the superiority of SR technology over the Merci device, there was no significant difference in near complete/complete reperfusion, final infarct volumes or clinical outcomes between SR and Penumbra thromboaspiration.
