E-249 Analysis of racial and accessibility disparities with mechanical thrombectomy usage for acute ischemic stroke at a single center
ConclusionRacial status was not associated with a difference in clinical presentation, treatment characteristics, nor outcomes. In multivariate analysis, stroke-onset-to-arrival time to our center was the greatest predictor of good neurological outcome. Racial status did not show any significant differences in stroke-onset-to-arrival times at our institution.Disclosures B. Abraham: None. C. Cunningham: None. R. Jaquish: None. W. Troja: None. G. Prediction of death after endovascular thrombectomy in the extended window: a secondary analysis of DEFUSE 3 "
Conclusions Despite selective inclusion criteria and overwhelming benefit for EVT, a substantial number of EVT patients in DEFUSE 3 died. The preprocedural variables age, NIHSS, wake-up stroke, and diabetes may predict this risk. Our predictive score provides a basis for future research to determine which factors influence lethal outcome after EVT.

P-036 Sensitivity of the RACW score in the detection of large vessel occlusions during working and non-working hours
ConclusionThe sensitivity of the RACE score for detecting large vessel occlusions does not vary significantly during working and non-working hours. However, patients who present with high RACE scores during working hours are more likely to have a diagnosis of a stroke mimic than those presenting off hours.Disclosures E. Hitomi: None. M. P-059 Race as a social determinant of poor outcomes following unruptured aneurysm surgery
ConclusionIn this nationwide analysis, racial disparities were present in that Black patients were at increased odds of poor outcomes and both Black and Hispanic patients experience longer lengths of stay when compared to White patients after adjusting for several possible confounding factors. In the shared effort to provide equitable care, an important first step is analyzing and acknowledging the differences in objective outcomes as racial disparities in neurosurgery. Understanding the specific factors underlying the differences, such as possible differences in care access, is the next step to improve health equity in ce...

P-008 Normoglycemia following thrombectomy is associated with improved 90-day favorable outcomes in ischemic stroke patients
Conclusions Normoglycemia is an important marker following thrombectomy. If patients have normoglycemia following thrombectomy and recanculization, they are likely to have a favorable long-term course. Hyperglycemia following thrombectomy however is an indication of poor future outcome. Further studies are warranted to elucidate the validity of normoglycemia as a marker for long-term favorable outcome following thrombectomy. Disclosures B. Lucke-Wold: None. S. Boo: None. J. E-049 Accuracy of non-invasive and invasive imaging in identifying ica occlusion: a comparative study in 65 patients
Conclusion Our result suggests that there is a lack of correlation between the imaging findings of carotid occlusion on imaging compared with catheter angiography of the ICA during the intervention in acute stroke treatment. Thus, endovascular catheterization of the ICA is often required for the exact characterization and localization of the carotid occlusion and to determine if a dissection is the underlying pathology in such cases. Factors associated with perforator stroke after selective basilar artery angioplasty or stenting
Conclusions Diabetes, time from last symptom to procedure, and pre-procedure stenosis percentage may be factors affecting the risk of perforator stroke associated with basilar artery angioplasty and/or stenting. These factors should be considered in planning of potential basilar artery angioplasty and/or stenting and prospectively evaluated in future multicenter trials.
