Abstract 144: Cost of Cardiovascular Disease Episodes among Patients with Hypertension Session Title: Poster Session I
Conclusions: Cost estimates of CVD episodes among hypertensive patients are consistent with results from the scarce literature in this area. Moreover, our study finds evidence of increased medical resource utilization weeks before the recording of the CVD episode. Omitting these pre-event costs leads to an underestimate of the true costs of CVD.

Abstract 152: Persistence of Warfarin versus Dabigatran Therapy in Patients with Atrial Fibrillation: Results from the ORBIT AF Registry Session Title: Poster Session I
Conclusion: Persistence rates for warfarin were higher at one year than those on dabigatran. In addition, factors associated with persistence of warfarin include: African American and Hispanic race, type of AF including permanent and persistent, LVH, and CHA2DS2-VASc risk scores ≥ 2.

Conclusions: Gender is an important prognostic factor in CVD, but is only included in about one third of published CPMs. Gender is much more frequently included as a predictor of incident CVD among the disease-free than of prognosis in those with established CVD.

Abstract 166: Developing the Veterans Affairs Cardiac Risk Score Session Title: Poster Session I
Conclusion: We demonstrated that an EHR in a specific population could risk-stratify patients as well those from as organized cohort studies and greatly improve calibration. Further, our finding that the ASCVD score greatly underpredicted in our population, while previous work have reported the ASCVD over-predictind in other cohorts, suggests that rather than arguing about which risk tool is best, our patients may be better served by us focusing on calibrating CV risk tools for our specific patient population using their EHR data.

Conclusions: There is an abundance of CPMs available for many CVD conditions, with substantial redundancy in the literature. The comparative performance of these models, the consistency of effects and risk estimates across models and the actual and potential clinical impact of this body of literature is poorly understood.

Abstract 206: Aiming to Improve Stroke Care Continuity with Primary Care Follow-up Appointments Scheduled Prior to Hospital Discharge Session Title: Poster Session II
Conclusions: Hospital performance with scheduling primary care follow-up appointments improved significantly; however, only 1 in 4 patients had an appointment scheduled prior to discharge. Case study analysis of missed opportunities may help identify barriers and facilitators associated with access, availability, and awareness that can be addressed in future improvement cycles.

Abstract 225: Frequency and Assessment of Poor Global Outcome in Patients with Left Ventricular Assist Devices Session Title: Poster Session II
Conclusion: In this large, single-center study assessing global outcome after LVAD implantation, we found that about a third of all patients had experienced a poor global outcome at 1 year. While LVAD therapy remains life-saving and the standard of care for many patients with advanced heart failure, these findings could help guide discussions with eligible patients and families. Future work should compare patients’ pre-LVAD expectations with likely outcomes and create risk models to estimate the probability of poorer outcomes for individual patients using pre-procedural factors.

Abstract 307: Gender Differences in Recurrent Cardiovascular Events Among High-risk Patients With Hyperlipidemia Session Title: Poster Session III
Conclusions: Among high-risk patients (i.e. history of CV events) enrolled in US health plans, women were more likely to have a subsequent CV event and multiple CV events sooner than men. Further research is needed to ascertain whether the gender differences might reflect differences in the intensity of treatment for hyperlipidemia between men and women.

Conclusions— β-blocker use in patients with prior MI but no heart failure was associated with a lower composite cardiovascular outcome driven by lower risk of recurrent MI with no difference in mortality. However, β-blocker use was not associated with lower cardiovascular events in those without MI, with a suggestion of inferior outcome with regard to stroke risk. Conclusions— AF is common clinical problem with significant morbidity and mortality. Get With The Guidelines-AFIB is a national hospital-based AF quality improvement program designed to increase adherence to evidence-based guidelines for AF.

Conclusion: This study represents the largest clinical experience of the safety of thrombolysis in patients on prior antiplatelet therapy. Despite a higher incidence of sICH, the absolute excess risk appears small (0.67%). These findings support current guideline recommendations regarding use of intravenous tPA in patients on antiplatelet therapy with careful weighing of potential risk and benefit.

Abstract 20: Public Reporting of Mortality Rates for Cardiovascular Conditions Did Not Improve Patient Outcomes Session Title: Concurrent II Session A: Oral Abstracts on Policy Issues
Conclusions: We found that mortality rates for publicly reported conditions were improving slightly during the period when only processes of care were being reported, but that these improvements slowed or reversed once public reporting of mortality rates began. These findings suggest that public reporting may be necessary, but is clearly not sufficient, to improve patient outcomes.
